Parental Authorization
(For Applicants Under 18) Parental Authorization for Underage Drivers (Form HSMV 71142)
Requirements:
Obligatory if you are younger than 18.
Must include the signature of a parent or legal guardian.
The original document must be presented (photocopies or scans are not accepted).
Driving Practice Verification
(For Minors & Practice Hour Compliance) Verification of Minor Driving Practice (Form HSMV 71143)
Requirements:
Applicants under 18 must complete 50 practice hours in total, including 10 hours at night, before being eligible for a Class E license.
Must be signed by a parent or supervising adult.
Adults starting to drive: Even though this form does not set certified practice hours, regular driving practice is highly recommended. Our professional lessons can help you build the necessary skills.
Residential Proof
(For All Applicants) Residence Verification (Form HSMV 71120)
Requirements:
Both minors and adults must submit two documents proving Florida residency.
Acceptable examples: utility bills, bank statements, lease agreements.
This form is used only if you cannot provide two standard proofs of address, in which case a Florida resident must certify your residence.
